PHARM-ASSIST is a Medication Therapeutic Management (MTM) program that can either be a comprehensive review of medication therapies or a targeted approach that looks at actual or potential medication related problems. In a comprehensive review, the pharmacist will review all prescription and nonprescription medications, herbal products, and other dietary supplements. The pharmacist will gather medical history and other pertinent information and evaluate patient's medications for the presence of any medication related problems, including adherence and patients thoughts and feelings about their condtions and medication use. Patients are evaluated for symptoms that could be attributed to adverse events caused by any of his or her current medications. Labs are interpreted, monitored and assessed. Any problems identified are prioritized and a plan for resolving each medication related problem is identified. Education on the appropriate use of medication and devices is provided. Communication of appropriate information is provided to the physician or other health care provider as are any recommendations. A Medication Record is provided and a Medication-Related Action Plan or list of items for the patient to act on in achieving specific health goas is provided.
