1. Clients are those who fill out all required paper work and have consultation with the provider in which a client-practitioner relationship is established. Purchases will be based on practitioner recommendations after a review of patient information and other requested data such as lab tests and questionnaires and consultation.

3. Disclaimer forms and HIPPA forms must be signed when indicated. You will be notified when this is required and the forms will be provided to you and must be signed and returned prior to receiving any of the services.
